Isosorbide dinitrate is a medication that belongs to a class of drugs called vasodilators. It works by relaxing and widening blood vessels, which helps improve blood flow and reduce the workload on the heart. This medication is primarily used to treat heart conditions such as angina (chest pain) and heart failure.
Uses:
- Unresponsive Left Ventricular Failure: This is a serious condition where the heart's left ventricle is unable to pump blood effectively. Isosorbide dinitrate can be used to treat this condition, especially when it occurs due to a heart attack.
- Unresponsive Left Ventricular Failure of Various Aetiologies: This refers to heart failure caused by various reasons, not just heart attacks. Isosorbide dinitrate can be used to treat this condition as well.
- Severe or Unstable Angina Pectoris: This is chest pain that is severe or occurs frequently. Isosorbide dinitrate can be used to relieve this pain and prevent it from occurring.
- Percutaneous Transluminal Coronary Angioplasty (PTCA): This is a procedure used to open blocked coronary arteries. Isosorbide dinitrate can be used to facilitate or prolong balloon inflation during PTCA.
Dosage and Administration:
Isosorbide dinitrate is a concentrated solution and should never be injected directly. It must be diluted before administration. Typically, a dose between 2mg and 12mg per hour is used, but doses up to 20mg per hour may be necessary. Prepared admixtures are always given by intravenous infusion or using a syringe pump incorporating a glass or rigid plastic syringe. The patient's blood pressure and pulse should be monitored closely during administration.
Important Precautions and Contraindications:
Isosorbide dinitrate should not be used in patients with known hypersensitivity to nitrates, severe anemia, cerebral hemorrhage, head trauma, diseases associated with increased intracranial pressure, hypovolemia, severe hypotension (systolic blood pressure less than 90mmHg), aortic and/or mitral valve stenosis, closed angle glaucoma, circulatory collapse, low filling pressure, cardiogenic shock, hypertrophic obstructive cardiomyopathy, constrictive pericarditis, or cardiac tamponade. It should also not be used in conjunction with phosphodiesterase type-5 inhibitors (e.g., sildenafil, tadalafil, vardenafil) due to the potential for severe hypotension.
Potential Side Effects:
Common side effects include headache, dizziness, orthostatic hypotension (low blood pressure when standing up), nausea, and flushing of the face. More severe side effects are less common but can include methaemoglobinaemia (a condition where the blood's ability to carry oxygen is reduced) and severe hypotension.
